If any personal information such as address, health or prescription medications has changed since your last visit, please fill out the the "Updated Established Patient Form" form below with the most up-to-date information. This will allow the doctors to make the most accurate decisions related to your care.
Health Insurance vs. Vision Insurance
There are two types of insurance that are accepted at our clinic: Health Insurance and Vision Insurance. As a general rule, if you are having a medical problem (conjunctivitis, retinal detachment, dry eye syndrome) with your eye, this would be billed to your health insurance. If you are coming in for a routine eye exam, this would be billed to your vision insurance.
We are in-network with Medicare, Medicaid, and most major health insurance plans.
